Depression with mixed features, also known as agitated depression or mixed-episode depression, is a mood disorder where individuals simultaneously experience symptoms of depression and mania or hypomania. This condition does not fully meet the criteria for bipolar disorder but presents a unique diagnostic challenge.

Though initially met with skepticism, the diagnosis of major depressive disorder with mixed features (MDD-MX) was officially recognized in 2013 within the DSM-5 and has since gained wider acceptance in the mental health community.

Symptoms of MDD-MX

Mood disorders are increasingly viewed as existing on a spectrum from depression to mania. Pure depression is now considered less common, with many individuals exhibiting symptoms across this range but leaning towards one side. Doctors assess where symptoms fall on this spectrum to differentiate between depression and bipolar disorder.

Depression with mixed features primarily involves depressive symptoms, such as persistent low mood and loss of interest, alongside certain manic symptoms like racing thoughts.

To be diagnosed with major depressive disorder, symptoms must persist daily for at least two weeks, including at least four of the following: fatigue, feelings of worthlessness or guilt, sleep disturbances, significant weight changes, psychomotor agitation or retardation, recurrent suicidal thoughts, and difficulty concentrating.

Diagnosing depression with mixed features is complex. Historically, manic symptoms indicated bipolar disorder, but it is now recognized that depressive disorders can include mild manic symptoms. Subtle hypomanic episodes often go unnoticed, complicating diagnosis.

Accurate diagnosis is crucial as treatment approaches differ significantly between bipolar disorder and depression with mixed features.

Treatment Options for MDD-MX

Treatment plans typically involve a combination of psychotherapy and medication. Antidepressants alone may be ineffective or worsen symptoms in some cases. Mood stabilizers and atypical antipsychotics such as Abilify, Geodon, Latuda, Rexulti, Saphris, Seroquel, and Zyprexa are often prescribed.

Medications like lithium or Depakote may be used at lower doses than for bipolar disorder.

For agitation or sleep difficulties, sedatives such as Ativan, Klonopin, or Xanax might be recommended.

In some cases, additional treatments like electroconvulsive therapy (ECT) may be necessary.
